Statins inhibit HMG CoA reductase, the rate-limiting step in cholesterol biosynthesis.798 This change produces a lowering of LDL-cholesterol levels.799-802 Inhibition of cholesterol synthesis reduces hepatic cholesterol content, resulting in increased expression of LDL receptors, which lowers serum LDL-cholesterol levels.803 Intermediate density lipoprotein IDL ; and VLDL remnants also are removed via the LDL receptor. The latter effect contributes to lowering of triglyceride-rich lipoproteins TGRLP ; by statins.86, 804, 805 Statins also appear to reduce hepatic release of lipoproteins into the circulation; 806, 807 this effect may be due in part to enhanced removal of lipoproteins by LDL receptors within hepatocytes or in the space of Disse.808 In some persons with homozygous familial hypercholesterolemia, high doses of statins lower LDLcholesterol levels.809-811 This latter action is mediated either by increased expression of residual LDL-receptor activity or by inhibition of lipoprotein assembly. The statins are generally administered with the evening meal or at bedtime. Somewhat greater LDL-cholesterol reductions occur when they are administered at night than in the morning. Most statins have a high first-pass clearance by the liver and a short half-life. Atorvastatin and its metabolites, in contrast, have very long halflives and thus morning administration is equally effective. Depending upon the specific statin and the dose administered, reductions in LDL cholesterol of 1855 percent are observed.812, 813 The reductions in LDL cholesterol are dose-dependent and log-linear, so that with each doubling of the dose of statin, LDL-cholesterol levels fall by about 6 percent. HDL cholesterol generally rises by 510 percent, but greater increases usually occur in persons with low HDL and elevated triglycerides.206, 207, 435, 436, The reductions in triglycerides with the statins generally range from 730 percent.206, 207, 416, 435, In individuals with triglyceride levels of 150 mg dL, triglyceride responses are inconsistent. But when triglyceride levels are 200 mg dL, triglycerides fall in direct proportion to LDL-cholesterol lowering.812 With very high triglyceride levels, however, LDL-cholesterol lowering is less than that observed with low triglyceride levels. The statins reduce the concentration of all LDL particles, including the small LDL particles, as well as IDL and VLDL remnants.86, 804 The combined lowering of LDL and TGRLP with the statins makes.

After dinner, we headed back to the hotel for more parking lot antics. On the ride back-even though all the muscle cars and vettes were behaving, the Doyle's & the Frangi's were discussing how disappointed they were since they weren't driving muscle cars that weekend. Well, I'm not sure what happened, but something bad & mean came over our quite, shy Ramone and he made one of the most beautiful burnouts I have ever seen in his F-250 diesel truck!!!! Well, as the story goes, he is now a legend a true hero to all the people of the swamp!!! News must spread like wild-fire or smoke ; in that small town and evidently Ramone had lit a fire in all of the red-neck, truck-driving locals!!! For over the next couple of hours I believe every youngster in that town who drives a truck, came to the Jameson to perform for their new idol, Ramone!!!! After watching about 50 local trucks do burnouts, we were growing bored and restless. The Dreamers really wanted some action from some pure muscle!!! Paul and his video guru sidekick, Ian, were the first ones up to the challenge. Paul cranked up "Black pearl"- his 68 Camaro also with a little red button ; which enticed "the Beast" to awaken, then Orange crush began to rumble and he encouraged his brother Toddie to unleash his overly powered '66 vette. All 4 of these muscle machines began terrorizing or might I say, "Challenging" rodders from adjacent hotels to come out and play. The excitement was too much for this town it was as though a demon seed had been awakened. Even a visiting "Granny Rodder" became envious and lustful when she learned of Ray's 75 yr old dad, enjoying his first burnout of his life in "the Beast"!!! -So, Granny went around the back of the hotel and emerged in her Malibu and laid some rubber!!! At the end of the nite, we were all suffering from smoke inhalation and were high from the fumes, so we turned in for the nite. Sunday morning, we made survey of the damage. I do believe if we had scraped all the rubber from the Jameson parking lot we could have collected enough to make a new set of tires!!!! Which I'm sure a few of the Dreamers will be purchasing on their return to Snellville!!!!!!! ; We were all nervous when checking out that morning--not sure of what the management would say or do.or if we would have to find a new hotel for next year, but instead they asked if they could go ahead and book our rooms for next year!!!! and we did--quickly, before they changed their minds!!! We will definitely be returning next year. It should be even bigger & better since it will mark their 30th anniversary of the event. We all had a wonderful time with our sister club and hope more of you will join us next year. On the way home, Robert explained to Ian that this past weekend should serve as a learning experience for him--titled, WHAT NOT TO DO 101!!!!!!!!!!!!!! In October, Michelle Duff had her hopes up. Maybe that drug that people with pancreatic cancer keep asking about will get approved in November. Duff is Director of Patient and Liaison Services PALS ; for the Pancreatic Cancer Action Network PanCAN ; , an organization that connects pancreatic cancer patients, health professionals and caregivers with information about the disease and available treatments. Over the last year or so, many have asked PanCAN about a drug called Tarceva. Tarceva is an oral drug first approved in November 2004 for the treatment of lung cancer. The FDA is considering also approving it to treat pancreatic cancer. A decision is due any time now. The timing couldn't be better. There hasn't been a new therapy approved for pancreatic cancer in nine years and Congress recently voted to designate November as National Pancreatic Cancer Awareness Month. The last treatment approved was Gemzar in 1996. Drugmaker OSI, in partnership with Genentech, is asking the FDA for an indication for Tarceva, in combination with Gemzar, for first-line treatment of patients with locally advanced, unresectable and or metastatic pancreatic cancer. It's a bit of positive news for a disease with a dark statistical image. According to the National Cancer Institute, it accounts for only 2% of all newly diagnosed cancers in the United States each year, but 5% of all cancer deaths. The disease is often far advanced by the time symptoms occur and the diagnosis is established. As indicated by five-year survival rates of less than 5%, successful treatment is rare. But it does happen. As with many cancers, there are those to whom statistics, numbers and averages do not apply. One of the free services of the PALS program is to connect people with pancreatic cancer with a volunteer who also happens to be a pancreatic cancer survivor. "It gives them a sense that there are survivors out there, " Duff said. "That they're doing just fine, they're living their lives, and they're feeling well." PALS tries to match the callers with survivors by gender, stage, history and age. PALS will also match caregivers with volunteers who will provide support and guidance. Of 100 volunteers, about 75 are survivors. The case managers also provide information on the disease, available treatments, symptom management and nutrition. Callers are connected with a PALS case manager immediately. The case manager follows up by sending a pack of information that is specific to the needs of a person diagnosed with pancreatic cancer. PALS will also conduct a personalized search for a clinical trial and send the results to the patient. PanCAN maintains a database of IRB approved clinical trials that are recruiting. "We want to make it as easy as possible for them, " Duff said. While the pace for approved new therapies for pancreatic cancer has been slow, Duff says the clinical trial options have increased in the last few years. "We're definitely seeing more treatments available in clinical trials, " she said. PanCAN provides these services free of charge. PanCAN is a not-for-profit organization. It receives about half of its funding from about 80 Team Hope grass-roots affiliates around the country that stage community fundraisers. Actually, from donor oocyte studies it is not clear whether there are any definite adverse environmental effects of the aged uterus causing implantation failure. Indeed, there are studies suggesting a decrease in PR and or implantation rates in donor oocyte programmes with advancing age Check et al., 1993a; Flamigni et al., 1993; Yaron et al., 1993; Cano et al., 1995; Rosenwaks et al., 1995; Borini et al., 1996; Levran et al., 1996 ; . However, many other studies have failed to demonstrate any reduction in fecundity with advancing age as long as oocytes from younger donors are used Rotsztejn and Asch, 1991; Sauer et al., 1992; Abdalla et al., 1993; Balmaceda et al., 1994; Navot et al., 1994; Legro et al., 1995; Remohi et al., 1997; Stolwijk et al., 1997 ; . One explanation for why different conclusions were reached regarding whether there is or is not an increase in uterine vascular impedance with ageing may relate to whether the women were studied on hormonal replacement therapy in preparation for having donor oocytes, or whether they were studied after ovarian stimulation. An earlier study Guanes et al., 1996 ; showed there to be no difference with the ageing uterus in donor oocyte recipients, while another study Cacciatore and Tiitinen, 1996 ; showed the benefits of ovarian stimulation in reducing uterine vascular impedance to be less apparent in the ageing uterus. The present study also evaluated the effect of ageing on uterine vascular impedance following ovarian stimulation, but failed to show any differences in younger versus older patients, in contrast to data reported by others Cacciatore and Tiitinen, 1996 ; . No differences were found in the sonographic assessment of endometrial thickness either on the day of retrieval or in mid-luteal phase. The possibility exists that differences in uterine vascular impedance are only evident on the day of embryo transfer the day studied by Cacciatore and Tiitinen, 1996 ; rather than on the day of oocyte retrieval, as in this study. However, we chose the day of retrieval because previous data suggested that improved implantation rates are correlated with reduced vascular impedance on this day Sterzik et al., 1989 ; , and this would be a more practical day to decide on whether to defer transfer in favour of cryopreservation of embryos. A significant decrease in impedance to uterine perfusion as a result of increasing progesterone concentration has been demonstrated, especially in the late luteal phase Bloechle et al., 1997 ; . Thus, it is possible that the day of embryo transfer is a superior time for measurements of uterine vascular impedance compared with the day of oocyte retrieval, in that it may be more important to see if progesterone fails to further improve uterine blood flow. However, if this hypothesis were true, one may have expected the mid-luteal phase Doppler sonography findings in the present study to reflect less of a 1682.

Conventional oximeters for whole blood do not assay for MetHb. Liposome encapsulated hemoglobin LEH ; was used as a synthetic hemoglobin fluids. Total transmittance and reflectance of the LEH suspensions were measured by spectrophotometer. OxyHb, deoxyHb and MetHb concentrations were calculated singular value decomposition SVD ; and were compared with known mixtures of [OxyHb]: [MetHb]. Also, diffuse reflection measurements were done in OxyHb, deoxyHb, MetHb, and mixtures of OxyHb-MetHb and OxyHb-deoxyHb. The constituent of hemoglobin derivatives analyzed by SVD. The mean deviation of the calculated concentrations from the "as mixed" values is 2 % for OxyHb and 16 % for MetHb. In addition, the effect of thermal incubation at 40 C LEH was determined. Our experiments stated that significant loss of OxyHb occurred after 4 hrs and all OxyHB was lost after 24 hrs. Also, singlet oxygen quenchers such as imidazole, sodium azide, and ascorbic acid were not protective against thermal incubation. But radical scavenger sodium formate ; and antioxidant agents -tocopherol ; caused protection against thermal effect when they were added to the lipid mixture. For example, for a LEH mixture consisting of 70% OxyHb, 10% deoxyHb, and 20% MetHb, reflection coefficient was and gleevec.
