As with coumarin, human lung microsomes are not particularly effective in metabolizing styrene to styrene oxide Carlson et al., 2000; Filser et al., 2002; Nakajima et al., 1994 ; , and metabolism of naphthalene to naphthalene oxide is much slower in human than in mouse whole lung microsomes Buckpitt et al., 1986 ; . Thus, it is hypothesized that the abundance and high activity of CYP2F2 in the mouse Clara cell predisposes this species and cell type to metabolism-dependent toxicity from xenobiotics such as coumarin, naphthalene and styrene Born et al., 2002; Buckpitt et al., 1995; Hynes et al., 1999 ; . Species differences in lung metabolic activity also appear to be driven in part by the significantly higher number of Clara cells within the terminal bronchioles of mice compared to rats and humans Lumsden et al., 1984; Plopper et al., 1980b ; . Furthermore, Clara cells differ morphologically among species; with human cells containing little smooth endoplasmic reticulum Plopper et al., 1980a ; . Based on metabolic, anatomical, and morphological differences between rodents and humans, and the apparent metabolic requirements for toxicity in the mouse lung, the human lung is unlikely to be susceptible to coumarin-induced Clara cell toxicity.

C3H HeJ mice feature a single point mutation in the Toll like receptor 4 gene which renders these animals resistant to a number of pro-inflammatory agents including lipopolysaccharide and ozone. This study compared pulmonary inflammatory responses in endotoxin resistant C3H HeJ ; and susceptible mice C3H OUJ ; after instillation of different size fractions coarse, fine and ultrafine ; of coal fly ash CFA ; or ambient air particles. Pulmonary neutrophils were increased in either strain exposed to the various sizes of ambient although no significant differences were evident across the treatment groups. Coarse and fine ambient did however cause a greater increase in TNF-alpha release and albumin levels in the lung fluid of endotoxin sensitive mice, suggesting a requirement for Tlr-4 receptors in these responses. Coarse and fine CFA also caused a greater increase in PMNs, albumin and cytokine levels in the endotoxin sensitive mice, however no differences were observed after exposure to ultrafine CFA despite high level of responses. We conclude that soluble markers of inflammation and edema induced by coarse and fine particles are associated with a functional Tlr-4 pathway, whereas responses induced by ultrafine particles from combusted coal are not Tlr-4-dependent. PMN influx may be Tlr-4 dependent or independent depending on the stimulus. This abstract does not reflect EPA policy.
The Hypertension Detection and Follow-up Program, 1979 The Hypertension Detection and Follow-up Program HDFP ; was a study that compared a stepped-care SC ; blood pressure BP ; lowering program with referred care RC ; in over 10, 000 men and women with diastolic hypertension of varying severity. The major aim of the study was to determine if more aggressive BP reduction, as compared with conventional treatment, could reduce major morbid and fatal outcomes. The study screened 158, 906 people to identify and enroll the target sample. Once enrolled, patients were randomized to SC or treatment options. The SC group was offered antihypertensive therapy in special centers and therapy was increased stepwise to achieve and maintain BP levels at or below the target level. * The SC therapy involved 4 steps. Step 1. Prescription of the diuretic chlorthalidone triamterene or spironolactone prescribed as supplementary or alternative Step 2. Addition of an antiadrenergic drug, preferably reserpine methyldopa prescribed alternatively Step 3. Addition of the vasodilator hydralazine; and Step 4. Addition of an antiadrenergic drug, guanethidine sulfate, with or without discontinuation of medication. Patients in the RC group were referred to their "usual sources" for treatment. Blood pressure remained consistently lower in the SC group, albeit on average only 5 mmHg lower. The study duration was 5 years. * diastolic BP DBP ; 90 mmHg for those entering with DBP of 100 mmHg or already receiving antihypertensive therapy, and a 10 mmHg decrease for those entering with DBP 90-99 mmHg.
