JONATHAN BATESON, DPhil Student Mr Bateson arrived in September 2002 as a first year DPhil student funded by the AHRB. His research is focused upon the development of morbid anatomy in England from the time of William Harvey until the publication of Giambattista Morgagni's De Sedibus et Causis Morbundum. Whilst revisionist accounts of the `birth of the clinic' have highlighted the work of London's anatomists in the late eighteenth century to argue for a more continuous account of the development of pathological anatomy, little attempt has been made to understand the heritage or development of this late eighteenth-century British pathology from the widely and systematically practiced discipline of morbid anatomy. Mr Bateson's research this year has explored both the initial intellectual impetus behind such research programmes and the social and institutional conditions which made such a practice possible and he will continue by exploring the fate of these research programmes and the way that they interacted with the social and intellectual changes in the eighteenth century, and in particular the effect of Morgagni's work. Mr Bateson presented papers both at the American Association for the History of Medicine annual meeting in Madison, Wisconsin, and at the BSHS conference in Manchester. LAURENCE BROCKLISS, Senior Research Associate Prof Brockliss has principally spent the last year in continuing his prosopographical study of British army and navy surgeons who served during the French wars, 17931815. This is a collaborative venture, which has been funded partly by the University of Oxford and partly by the Wellcome Trust. The manuscripts of two books arising from the project are now virtually complete. The first, entitled Advancing with the Army: Professionalisation and Social Mobility in the Army Medical Corps, 17901850, will be delivered to OUP in January 2005. The second, a scholarly biography of Nelson's surgeon, Sir William Beatty, will be published by OUP, or a commercial press, in time for the 200th anniversary of the Battle of Trafalgar. Prof Brockliss and his research assistant, John Cardwell, have also been closely involved during the past year in the making of a TV documentary about Beatty, which has again been partly funded by the Wellcome Trust. Prof Brockliss continues to work on early-modern French medicine and recently gave a paper on the content of the libraries of French physicians to a conference at Cambridge on religion and medicine in the Enlightenment organised by Ole Grell and Andrew Cunningham. He is also editing the autobiography of a Montpellier physician, Pierre-Joseph Amoreux, with the help of a former graduate student of the unit, Amanda Berry, which it is hoped will be placed on the web by Pietro Corsi.

Distraction view Several Authors have discussed the usefulness of stress radiographs of the hip to evaluate joint laxity. However, in 1990, G. Smith and colleagues at the University of Pennsylvania described an improved method of distracting the joint as well as calculating its laxity14. With this distraction technique, the hip is held in a neutral position to avoid twisting of the joint capsule, which would limit the distraction effect; to calculate the joint laxity, the distraction index DI ; , which is calculated by dividing the measured distance between the femoral head centre and acetabular centre by the radius of the femoral head, is used14. This yields a unit less variable and with a range from 0 to 114. A hip with an index close to 0 is tightly compressed in its congruent position, and a hip with an index close to 1 has little or no acetabular coverage of the femoral head i.e., joint luxation; 14 ; . A distraction index of 0.30 is considered to be the dividing line between normal hips and hips predisposed to hip dysplasia3, 14, 15, although breed variations have been described. Some dogs with a distraction index greater than 0.3 do not develop hip dysplasia when the joint laxity is caused by a muscular imbalance between the abductors and adductors only and is not dependant on an excessive slope of the DAR3, 10. Some dogs with hip dysplasia have a distraction index of less than 0.3 because capsular fibrosis, following inflammatory arthritis, limits femoral head luxation. In a study conducted by R. Badertscher at the University of Georgia in 1977, the half-axial position was used to improve the radiographic visualization of subluxation in dogs with hip dysplasia; the results were similar to those described later by Smith 1990, 1998 ; . We have used the procedure described by Badertscher since 1994 because it is simple, rapid and provides reliable; the only modification we made has been improvement of the device used to distract the hips. The dog is positioned ventrodorsally with the femurs slightly extended at approximately 100-110 to the table surface and the tibiae parallel to the plane of the table surface 1. For this purpose, we developed an S-shaped teflon table-device that is 2 cm thick, 5 to 12 cm wide and 50 cm long; the S-shape provides good adaption to the pubic area. The device has a hinged base to keep it in contact with, and parallel to, the table, and is placed on the ventral surface of the pelvis. Pressure applied to the medial aspects of the proximal femurs through the hand-held tibiae results in the fulcrum to subluxate the femoral heads from the acetabula1. To compensate for the superimposed muscle mass, an increase of 10 kV necessary for adequate tissue penetration of the Xrays1. In our experience, slight hip extension does not cause twisting of the hip joint capsule to an extent that limits complete hip distraction obtained with the hip in a completely neutral position; consistent results were obtained in the same dogs using both positions. SPONTANEOUS PROGRESSION OF CONSERVATIVELY-TREATED CASES By monitoring conservatively-managed cases that had an early diagnosis but no surgical treatment, it was possible to correlate early diagnostic findings to the development of CHD and to the severity of osteoarthritis at the end of the growing period or later in life. It was possible to perform a reliable retrospective study, which compared the degree of naturally progressing CHD and osteoarthritis with the early diagnostic findings, by using consistent evaluation methods.
