In the five patients with clinical manifestations due to L-OHP HR, prick tests gave negative results. Intradermal tests with L-OHP elicited an immediate positive reaction in four out of five patients. Prick tests and intradermal tests performed with solutions of cisplatin, carboplatin, and the two Pt salts all gave negative results. In the patient with a negative intradermal skin test, L-OHP rechallenge was attempted, but it resulted in a severe HR, which required the patient's admission to hospital. A subsequent prick test was performed, which now became positive. In all five patients, L-OHP was stopped and the patients continued therapy with 5-FU and FA without any further problems Table 2 ; . The same tests were carried out in 15 other subjects undergoing therapy with 5-FUFAL-OHP without any skin reactions: no false-positive test was found. Through this experience, we have shown that an intradermal skin test with L-OHP confirmed the clinical diagnosis of L-OHP HR in four out of five patients, 80%. The test was negative in all the 15 patients with no previous history of L-OHP HR. The characteristics of the clinical manifestations suggest a type 1 HR, i.e. IgE mediated. The mechanisms by which L-OHP can cause HR need to be established. We did not test for specific anti-Pt antibodies, 1, 2-diaminocyclohexane or oxalic acid. The lack of HR to other Pt salts seems to indicate that the whole molecule of L-OHP, rather than just the Pt, plays a pivotal role in eliciting immediate HR. We have previously demonstrated that an immunological response is the basis of haemolytic anaemia caused by L-OHP [8], and it may be speculated that the binding of L-OHP to cutaneous receptors alters or modifies some ligands and promotes an immunological response. These data partly confirm the findings described by Meyer et al. [5]. They report a cross-reactivity with carboplatin, which was not the case in our series. Moreover, they stated that L-OHP reintroduction did not lead to any severe reaction, whereas, in our only patient with a false and halofantrine.

Scientists will discuss their studies of genetically isolated populations -- European Gypsies, Norfolk Islanders, New Zealand Maoris, and Finns -- at a media conference at this week's International Congress of Human Genetics. The studies look at how an understanding of common genetic diseases in smaller populations can benefit the greater human population. Isolated populations provide scientists with a concentration of information so that they can easily sample and compare genetic information. From the results, we can glean more about our history and predisposition to genetic diseases. Professor Leena Peltonen is among leading molecular geneticists and the current president of the Human Genome Organisation. She is a pioneer in the use of genetically isolated populations to identify disease genes. `We have identified globally important genes behind high cholesterol and triglycerides, multiple sclerosis and schizophrenia using a special population of Finland, ' says Professor Peltonen. `The population has provided a short cut for identifying new disease genes and pathways, relevant for all people' `A single migration wave out of India around 1000 years ago has given rise to the current population of European gypsies', says Professor Luba Kalaydjieva of the Western Australian Institute for Medical Research. `We have realised that you can't go very far in discovering genetic structure and disease without understanding the cultural anthropology and social structure of a population', Profesor Kalaydjieva continues. `There are close parallels between Gypsies and Ashkenazi Jews in both their social history and its biological consequences, and looking at the similarities and differences is fascinating.' Exercise should be a part of the care of all older patients troubled by chronic pain. A. Initial training should be conducted over 8 to 12 weeks and should be supervised by a trained professional with knowledge of the special needs of older adults. B. Exercise should be tailored to the needs and preferences of the patient in consultation with the primary clinician. C. Moderate levels of exercise conditioning aerobic or resistance training ; should be maintained indefinitely. VI. A trial of physical or occupational therapy is appropriate for the rehabilitation of impaired range of motion, specific muscle weakness, or other physical impairments associated with chronic pain. VII. Traditional insight-oriented psychotherapy should not be used alone for the management of chronic pain. VIII. Other nonpharmacologic therapies may be helpful for some patients with chronic pain. A. Chiropractic, acupuncture, or transcutaneous nerve stimulation may be helpful for some patients, but they are expensive and have not been shown to have greater benefit than placebo controls in the management of chronic pain. These interventions should be provided only by professionals. B. Self-administered heat, cold, and massage and the use of liniments and other topical agents may be helpful for some patients. 1. Initial instruction and demonstration should be provided by a trained clinician. 2. Precautions against thermal injury should be provided, especially for patients with sensory disturbances e.g., diabetic patients ; or with cognitive impairment. 3. Patients should be cautioned about the toxicity of or possible reactions to linaments and other topical agents.
