The patient was a 36-year-old female who had tripped at the trailhead to Avalanche Peak while preparing for a hike on the morning of admission. I called the attention of the provider to the fi lms, and he agreed with the assessment based on the patient's history, physical exam and X-ray fi ndings. He examined the patient, and then directed me to splint the injured foot and make arrangements for the patient to follow-up with an orthopedics specialist at the closest hospital. Using traditional plaster casting materials and Ace bandages, I immobilized the foot with a posterior mold splint. While the splint cured, I worked with the nursing staff to arrange for appropriate follow-up, and addressed the patient's needs for pain control, use of crutches, and discharge instructions. Twenty-four hours later, I stood atop Mt. Sheridan at 10, 500 feet, overlooking some of the most remote wilderness in the continental United States. In July, 2004, I had a tremendous opportunity to combine education with recreation during a fourth-year clinical rotation at Yellowstone National Park.
