The patient was a 36-year-old female who had tripped at the trailhead to Avalanche Peak while preparing for a hike on the morning of admission. She reported that she had inverted her right ankle, and suffered immediate pain and inability to bear weight. She had no other injuries, and no other relevant medical history. The clinic was rather busy when I assessed the patient. Given the mechanism of her injury, and the physical exam fi ndings of tenderness, ecchymosis and swelling, it was apparent that an X-ray would be required per the Ottawa ankle and foot rules. Since the staff was preoccupied with other patients, I wheeled the patient into the X-ray room myself, positioned her appropriately and took AP, lateral, and oblique plain fi lm radiographs of her injured foot and ankle. While she waited, I slipped into the darkroom, and using the equipment available, developed the X-ray fi lms in a short period of time. When I was fi nished, I placed them up on the light box, and immediately identified a fracture just distal to the head of the 5th metatarsal--a Jones fracture. I called the attention of the provider to the fi lms, and he agreed with the assessment based on the patient's history, physical exam and X-ray fi ndings. He examined the patient, and then directed me to splint the injured foot and make arrangements for the patient to follow-up with an orthopedics specialist at the closest hospital. Using traditional plaster casting materials and Ace bandages, I immobilized the foot with a posterior mold splint. While the splint cured, I worked with the nursing staff to arrange for appropriate follow-up, and addressed the patient's needs for pain control, use of crutches, and discharge instructions. Twenty-four hours later, I stood atop Mt. Sheridan at 10, 500 feet, overlooking some of the most remote wilderness in the continental United States. In July, 2004, I had a tremendous opportunity to combine education with recreation during a fourth-year clinical rotation at Yellowstone National Park. It was an incredible experience that gave me the opportunity to explore a variety of aspects in emergency, rural, and wilderness medicine. Yellowstone National Park is one of the crown jewels of our National Park System. It also holds the honor of being our country's fi rst national park, established by the United States Congress in 1872. Yellowstone is a land of sublime beauty and majesty, with vast emerald colored lakes, sprawling alpine meadows, intricately carved canyons, dynamic hydrothermal wonders, and towering mountain peaks. Beyond these natural spectacles, Yellowstone is home to some of the most magnificent animals in North America, including elk, bison, bald eagles, grizzly bears, and wolves. In traveling to various national parks throughout this country, from Biscayne and the Everglades in Florida, to Denali in Alaska, I can think of no park that has the sheer variety of landscape, and the flora and fauna of Yellowstone. That would immediately give rise to difficulties caused by constitutional differences between the two countries, in particular the different understanding of the separation of powers principle, together with the fact that, unlike the Netherlands, Switzerland does not have a single Conference of Prosecutors General for the whole country but instead has twenty-six autonomous cantonal prosecutors who are not normally beholden to any authority and at any event do not take orders from the Federal Minister of Justice ; . What is more, the power to waive prosecution across the board for certain offences would constitute a de facto disapplication of the substantive law and consequently would amount to a transfer of legislative power from the legislature to the body empowered to issue the relevant guidelines. These difficulties do not mean that a system equivalent in its end effect to the Dutch model is not possible, but they would have to be taken into account in the way the system was bis designed.
