Most patients with diabetes will require insulin therapy while receiving parenteral nutrition PN ; . For those already on insulin therapy, the insulin dose requirement will generally increase. The insulin therapy can be administered via either an intravenous variable rate insulin infusion, or via subcutaneous injections, using either twice daily Isophane insulins or a 30 pre mixed insulin mixture, see the green chart ; . If glycaemic control proves particularly difficult on a twice daily subcutaneous insulin regime, then the patient should be switched to a basal bolus insulin regime, administering soluble insulins three times daily, and Novorapid or Humalog ; Insulin glargine or detemir insulin at bed time. Patients on longer term PN are often changed to night-time feeding usually 12-14 hours ; . Insulin regimens will need to be altered accordingly, and the nutrition team will need to liaise with the Diabetes Nurse Specialists. B ; MAINTAINING GLYCAEMIC CONTROL Poor glycaemic control is linked to an increased risk of nosocomial infections, and thus an increased risk of catheter related infections. Insulin doses will need to be adjusted according to any changes in the carbohydrate content of the parenteral feed. Blood glucose levels should be closely monitored 4 hourly initially, and 6 hourly once stabilised ; . For patients requiring intravenous variable rate insulin infusion, the green UHL intravenous insulin therapy charts should be employed, and the insulin dose for each blood glucose level should be adjusted according to response. KEY POINTS For patients receiving enteral and parenteral nutrition, extremes of glycaemia should be avoided and target blood glucose levels should be between 5 and 8 mmol l. Patients with diabetes who are commenced on enteral or parenteral feed will usually require an increase in their diabetes medication. This invariably means either a commencement of insulin therapy or an increase in insulin doses. If a patient on enteral or parenteral nutrition becomes hyperglycaemic, then the diabetes therapy needs adjusting, rather than a reduction in nutrition. This usually requires an increase in the insulin dose. Communication between the Diabetes Nurse Specialists and the Nutrition Support Dietitian Nutrition Team is vital, and the targets for blood glucose control should be established for the individual patient, avoiding hypoglycaemia in "at risk" patients The decision to commence enteral or parenteral nutrition is not affected by the patient having diabetes, and the avoidance of over feeding is of more importance than which formula is used. Her prognosis is also somewhat dependent on her status in her workplace and her ability to modify tasks or successful candidacy in obtaining a promotion. Recommendations: The overall recommendation is for Ms. Dewhurst to be discharged home to pursue work at a medium level for an eight hour day within the restrictions for squatting and lifting. Supporting recommendations include participation in a gym and pool program independently for the next three months. We would also suggest that she purchase knee pads, and the use of a theraball for her home exercise program. We would suggest that Ms. Dewhurst follow up with her family doctor related to shoulder and knee pain which are non-MVA related conditions.28.
