Purpose. To study the impact of excipient selection on the properties drug morphology, dissolution, stability, etc. ; of ketoprofen containing formulations produced via hot melt extrusion. Methods. Ketoprofen USP classified as very slightly soluble ; was selected for study. Polymeric excipients evaluated were ethylcellulose ETHOCEL STD 10 ; , hypromellose METHOCEL E5 ; , polyethylene oxide POLYOX WSR N-10, 1105 ; , as well as some proprietary materials. Extrusion was performed on both a single screw extruder and twin screw extruder. Extruded rod was cut to produce tablets. The tablets were characterized via Differential Scanning Calorimetry DSC ; and X-ray Crystallography. Dissolution studies were performed in Phosphate buffer pH 7.4 ; . Accelerated stability studies were conducted at 40C, 75% RH. Results. DSC and X-Ray results showed that the choice of excipient has a significant impact on the crystalline nature of ketoprofen in the extrudate. For example, a formulation comprising 20% ketoprofen in 80% POLYOX N-10 exhibited no ketoprofen crystallinity. Similar results were found for ETHOCEL STD 10 and formulations containing other proprietary excipients. Conversely, a formulation produced with METHOCEL E5 clearly exhibited the presence of crystalline ketoprofen. Dissolution studies performed with the ketoprofen POLYOX N-10 composition showed 100% ketoprofen release by 200 minutes. Dissolution of samples subjected to accelerated stability testing show similar results after 1 and 3 months. Dissolution stability results were found to vary with the polymeric excipient used. Conclusion. Excipient selection plays an important role in the characteristics of dosage forms produced by Hot Melt Extrusion. Extruded compositions of ketoprofen and POLYOX N-10 contained no detectable crystalline ketoprofen and exhibited stability at accelerated conditions. Stability performance was found to vary with excipient choice and klonopin.

Using a multistage systematic random sampling technique and internet generated random numbers, one state was selected in each of the six geo-political zones in the country. In the six randomly selected states, the state capital was chosen and the largest health facility which was usually either a tertiary or secondary health facility was first sampled. Six local government areas which have private pharmacies were also identified and sampled. In anticipation of nonfunctioning facilities and other unforeseen problems in the field, four extra facilities of each type were sampled and data collectors were requested to seek approval before collecting data from the reserve facilities. In view of the methodology employed in sampling the facilities included in the study, the result can be generalized to the whole country. Data collectors were also sent to the State Central Medical Stores in each of the states sampled from the six geo-political zones. Essentially, the WFSA is a federation of societies of anaesthesiologists as its name suggests whilst World Anaesthesia is a group of individuals with similar aims, namely "to make available the highest standards of anaesthesia to all the peoples of the world." Naturally, the two organisations work closely together and try to support each other's endeavours. This issue of World Anaesthesia News continues the tradition of publishing articles from individuals and societies in developing countries. At the end of these articles from Ethiopia, Eritrea and Zambia ; I have added a few statistics on gross domestic product GDP ; , infant mortality and life expectancy. The figures are all too familiar: GDP is under US 00 per capita, infant mortality approaches 100 1, 000 and life expectancy is less than 50 years. In the developed western world, comparable figures are GDP -30, 000, infant mortality 5-6 1, 000 and life expectancy 70-80 years. The figures for parts of Eastern Europe and the former USSR lie somewhere in between. With so little available to be spent on health, it is easy to be pessimistic but all the authors remain optimistic about the future in their countries I would also commend to you Prof. Thara Tritrakarn's essay, based on a talk he delivered at the World Congress in Montreal in June. He concludes that if we in the West can help train a single anaesthetist who returns to his or her home country, we will have made an immeasurable contribution to the improvement in health care in that country.
