Transient energy growth of 2D and 3D optimal linear perturbations for flow over a backward-facing step. Reynolds numbers based on the step height and peak inflow speed are considered in the range 0 Re 500, well below the critical value for the onset of absolute instability. This analysis quantifies for the first time the transient linear response of the flow due to local convective instability downstream of the step edge. The maximum linear transient energy growth is of order 80 103 at Re 500. The critical Reynolds number below which all perturbations decay in energy norm is Re 57.7. The centroidal location of the energy distribution for maximum transient growth is typically downstream of all the stagnation reattachment points of the steady base flow. While their growths are larger, the 3D optimal disturbances are broadly similar in shape to the 2D cases, and the corresponding spanwise wavelengths are of order ten step heights. Nonlinearity is shown to have a stabilizing effect on the instability.

There were no pregnancies with IVF, however there was successful fertilization following ICSI in six patients with two resultant pregnancies. However, both subzonal insemination and ICSI require expertise not available in all units and there are concerns over costs and outcomes. IVF has been previously tried as a treatment for ICS with no success Chemes, 1993 ; . Our case is the first documented successful IVF treatment of ICS. Unfortunately, only one oocyte was obtained and therefore we are unable to comment on the fertilization rate using this patient's spermatozoa. It is difficult to understand how spermatozoa with such severe axonemal disruption and impaired motility can penetrate the zona pellucida. Paternity was not proven directly in this case, however routine IVF safety procedures were performed by the embryologist to ensure correct paternity. It could be argued that not all spermatozoa were affected by axonemal disruption in this case. However, in all spermatozoa assessed at repeated electron microscopy there were axonemal defects. When assessed manually and with computer-assisted motility analysis, there were greatly reduced mean motility parameters total motility, progressive motility, mean path velocity, tract speed, lateral amplitude and beat frequency ; . There was overlap between the sample population and the normal range, and it is therefore possible that a few spermatozoa had motility parameters within the normal range. However, no spermatozoa displayed hyperactivation. It is possible that despite no hyperactivated motility spermatozoa can penetrate the zona pellucida. Lastly in view of the heterogeneous nature of Kartagener's syndrome, it is difficult to be certain that this case truly has Kartagener's syndrome rather than severe asthenozoospermia with other abnormalities being coincidental. However, this seems unlikely as all the clinical features and most of the laboratory features of Kartagener's syndrome are present in this patient. There are many incongruities concerning male infertility in ICS. Spontaneous pregnancies have been reported in 12 men affected by Kartagener's syndrome, though paternity was not proven Rott et al., 1979 ; . Kartagener's syndrome has been diagnosed in men with normal ciliary and flagellar structure Greenstone et al., 1985; Escudier et al., 1987 ; , in cilia without dynein arms and normal flagella Jonsson et al., 1982 ; and in flagella without dynein arms and normal cilia Wilton et al., 1986 ; . Blood toluene concentrations and clinical features of toxicity Oliver 1984 ; reported that 50 per cent of patients with blood toluene concentrations between 5 and 10 mg l showed marked intoxication, and that virtually all patients with concentrations greater than 10 mg l were comatose or dead at the time of sampling. Shortly after exposure, however, only signs of moderate intoxication e.g. slurred speech, unsteady movements ; have been associated with blood toluene concentrations as high as 30 mg l Garriott et al. 1981 ; . Blood toluene concentrations in samples from 132 patients who were thought to have engaged in volatile substance abuse ranged from 0.2 to 70 mg l and were in excess of 5 mg l in 22 of the 25 deaths reported Meredith et al. 1989 ; . Although there was a broad correlation between blood concentration and severity of poisoning, there were large variations within each patient group. Aside from individual differences in tolerance and possible loss of toluene from the sample prior to analysis Gill et al. 1988 ; , the lack of a strong correlation between blood concentrations and clinical features of poisoning is probably due to rapid initial tissue distribution and elimination. In the occupational setting, blood toluene concentrations after exposure to up to 127 parts per million toluene United Kingdom Occupational Exposure Limit 100 parts per million ; for 8 hours ranged between 0.4 to 6.7 mg l Campbell et al. 1987 ; . Some 80 per cent of a dose of toluene is converted to hippuric acid see Table 4 ; . Similarly, more than 90 per cent of a dose of xylene is metabolized to methylhippuric toluric ; acids. The principal isomer found in urine is 3-methylhippurate since m-xylene is the principal component of technical grade xylene see Table 1 ; . Methylhippurates are not normal urinary constituents, but hippuric acid may arise from the metabolism of benzoates in foods and medicines, and thus caution is needed in the interpretation of analytical results. Hippurate and methylhippurate excretion are often expressed as a ratio to creatinine since this obviates the need for timed urines. Occupational exposure to toluene can give rise to ratios of 1 gram of hippurate per gram of creatinine or more; in patients suspected of volatile substance abuse a ratio of more than 1 g g suggests, but does not prove, toluene exposure Meredith et al. 1989 ; . Measurement of urinary o-cresol has been proposed as an alternative means of monitoring toluene exposure selectively, particularly in occupational circumstances, but the assay procedure is relatively complex and is thus not widely used. 4.7.2. But, as Annas has pointed out, while many people `love babies and technologies and most . applaud the ability of the new assisted reproduction techniques to help infertile couples have children . a bad way to protect the children who have been conceived and born with the assistance of the new reproductive techniques is simply to provide the adults involved with what they want.' We have shifted the emphasis in adoption practices from the rights of the biological parents to the welfare of the children and we must do the same in relation to the use of new reproductive technologies. Objective--The role of Rho activation in the regulation of tissue factor TF ; is not clear. This study was undertaken to investigate this in endothelial cells induced by monocyte adhesion. Methods and Results--Isolated human peripheral blood monocytes were added to cultured human coronary endothelial cells. Monocyte adhesion to endothelial cells increased the levels of TF antigen in the endothelial cells. The results of transient transfection of the human TF promoter luciferase gene into endothelial cells indicated that the increase in endothelial expression of the TF gene caused by monocyte adhesion occurred at the transcriptional level. The upregulation of TF was inhibited by statins, and the suppressive effect of statins was reversed by geranylgeranylpyrophosphate. Monocyte adhesion rapidly upregulated the membrane translocation and GTP GDP exchange of RhoA, but not of Cdc42 or Rac, in endothelial cells. Rho inhibition by C3 exoenzyme or adenovirus-mediated expression of N19RhoA prevented the endothelial upregulation of TF caused by monocyte adhesion, and this was mimicked by Rho-kinase inhibitors. Moreover, monocyte adhesion increased the phosphorylation of nuclear factor- B p65 in endothelial cells, and this was prevented by statins and Rho inhibition. Conclusions--Our study shows that RhoA activation plays an integral role in TF expression in endothelial cells.
