Further, many of the same signaling pathways are activated in retinal and cerebral ischemia 1820 ; . Given the similarities between these two types of neuronal injury, we conducted a parallel study of the EPO EPO-R system in a model of retinal ischemia-reperfusion injury. EPO is expressed in the human fetal retina 21 ; , but it is unknown whether its expression persists in the adult and whether it has a physiological role in the retina. Using immunohistochemistry and Western blot studies, we observed that EPO-R is increased significantly in the retina by ischemia, which suggests that the ischemic retina can up-regulate recovery pathways that might be enhanced by the exogenous application of EPO. Indeed, systemically administered recombinant human EPO rhEPO ; pretreatment or posttreatment is associated with both histopathological and functional protection of retinal neurons subjected to ischemic injury. We further show that rhEPO inhibits apoptosis after acute retinal ischemia. The present study provides further evidence for a neuroprotective and antiapoptotic effect of EPO EPO-R and suggests that up-regulating this cytokine system may enhance recovery from ocular diseases involving acute neuronal injuries. Creased with exercise was similar to that of patients who exhibited an increase in ejection fraction with exercise and because no relationship could be established between age and left ventricular exercise reserve in our patients. 3-Adrenergic blockade may conceal deterioration of left ventricular systolic function in patients with coronary artery disease.24 25 However, this is also an unlikely possibility because the proportion of the patients who were taking 3-adrenergic-blocking drugs was not significantly different between the two groups 75% vs 53%; p NS ; . Moreover, the exercise protocols which might influence ejection fraction response to exercise, were similar in the two groups.26 Although it may be argued that two different modes of exercise were used for measurement of V02max and ejection fraction, MVO2 during supine bicycle exercise was unlikely to be lower than that with maximal treadmill exercise because the rate pressure product was higher with bicycle ergometer exercise. In addition, left ventricular end-diastolic volume, another major determinant of MVO2, is expected to be larger with supine than with upright exercise. True V0, max can generally be measured during a treadmill exercise test, ' which is not a suitable method for assessment of left ventricular function during strenuous exercise. Even maximal attainable V02 during upright bicycle exercise is generally lower than that during treadmill exercise in healthy subjects as well as in patients with coronary artery disease. 21 Thus, although exercise intensity in terms of total body VO2 was lower during the radionuclide studies than during the treadmill test, myocardial energy expenditure and MVO0 were probably not different between the two modes of exercise. Previous investigators did not find any relationship between maximal aerobic exercise capacity and left ventricular systolic function at rest or at peak exercise.` Although our results confirm some of the previously reported results, namely that left ventricular contractile function at rest has no appreciable effect on maximal aerobic exercise capacity, 28 we found a strong correlation between left ventricular exercise reserve and VO2max and a significant, although weak, correlation between peak exercise ejection fraction and V02max. The apparent disparity between our findings and those of others is probably because of the following: In this study we included only those patients who attained true V02max as defined by objective criteria and protopic.
