In addition, a total of five patients were withdrawn from the study: there were four withdrawals in the bosentan epoprostenol group two deaths due to cardiopulmonary failure, one clinical worsening, one adverse event due to elevated levels of hepatic transaminases ; and one withdrawal in the placebo epoprostenol group adverse event due to elevated levels of hepatic transaminases.
With primary pulmonary hypertension.1 6 Similar clinical effects of epoprostenol have been reported in patients with pulmonary hypertension associated with connective tissue disease7, 8 and congenital heart defects.9 The treatment therefore appears indicated in pulmonary arterial hypertension PAH ; as recently defined by a World Health Organizationsponsored consensus conference.10 However, due to its short half-life and chemical instability, long-term epoprostenol therapy requires a permanently implanted central venous catheter and a portable infusion pump, exposing the patients to a series of complications including catheter-related embolism or thrombosis, infection, and delivery system malfunctions resulting in poorly tolerated, rapid overdosing or underdosing.19 Accordingly, alternative modes of administration and longer acting prostacyclin derivatives are currently under investigation. Favorable results have been reported with treprosCHEST 121 5 MAY, 2002.
The Group applies a Return on Assets Employed "ROA" ; measure to evaluate the performance of its business segments. In 2001, overall Group ROA fell to 6.5% from 7.5% in 2000. ROA of Leisure had improved due to better utilisation of assets while those of Healthcare, Property and Investments declined as a result of lower profits.
The results of this clinical trial demonstrate that long-term inhaled administration of aerosolized iloprost, a stable analogue of prostacyclin, improves a clinically important combined end point consisting of exercise capacity, NYHA class, and clinical deterioration in patients with selected forms of pulmonary arterial hypertension and chronic thromboembolic pulmonary hypertension. Moreover, iloprost improved several secondary end points. Since intravenous epoprostenol was shown to improve survival among the most severely ill patients with primary pulmonary hypertension, it has been unethical to perform randomized clinical trials among patients with pulmonary hypertension in which survival is used as an end point. We chose a combined rather than a single end point e.g., the distance walked in six minutes ; in order to make a more rigorous determination of whether inhaled iloprost was efficacious. Nearly 40 percent of all patients who were treated with iloprost increased their six-minute walk.

Which patients should not be treated? `Active intravenous drug users should not be treated due to the risk of reinfection. In addition, compliance with treatment is poor in patients in whom alcoholism has not been interrupted and in whom drug addiction continues.'.

In the patient sample of the current study, starting combination therapy with oral sildenafil on top of chronic inhaled prostanoid treatment leads to a significant and sustained haemodynamic and functional improvement without significant side-effects. However, further additional therapy including i.v. epoprostenol might be required when liver transplantation is planned or patients are not stable on combination therapy [1]. There are major limitations in the current study. The rather low number of patients with heterogeneous aetiology of liver disease might indicate a selection bias. Subgroup analyses or potential side-effects require further investigation. Although the current study was not sufficient to deduce general treatment recommendations, the subjects were and esmolol.

The data are fed into a spreadsheet on a personal computer and conversions and basic statistics are computed automatically. The system consists of an AT-compatible PC with two serial interfaces and an analogdigital conversion board. Epoprostenol inhaled
In the few studies in which patients were randomized, patients were treated independently of the short-term responses to epoprostenol during dose-ranging [30, 86, 90]. However, long-term beneficial effects, particularly on survival, were commonly seen in patients in whom no short- or long-term haemodynamic changes were manifested with epoprostenol [86, 90]. Thus, the long-term effects of epoprostenol in PPH may be only partially related to its vasodilator properties, and may be due, at least in part, to increased systemic oxygen transport [90], or to other as yet unknown effects on vascular growth and remodelling, e.g. the formation of new plexiform lesions, or platelet-endothelium interaction [83, 85, 100]. Anticoagulation It has not yet been proved that thrombosis in small pulmonary vessels plays a role in the pathogenesis of PPH. WAGENVOORT and WAGENVOORT [101] analysed postmortem lungs from 156 patients with PPH. They found that in 31 of the 156 patients organized thrombi were located within small pulmonary arteries. It was concluded from these findings that thrombosis may be an important mechanism in the pathogenesis of PPH. In agreement with this concept is the clinical observation that anticoagulation appears to be effective in patients with PPH. A number of studies [48, 94] have meanwhile provided evidence that anticoagulation prolongs life in this patient group for example, RICH et al. [48] reported 47 versus 31% 5 year survival ; . Practice of oral anticoagulation. Oral anticoagulation with coumadines is the therapy of choice. Because of the frequent occurrence of chronic RVF and liver congestion, with subsequent fall in the plasma levels of circulating coagulation factors, the desired International Normalized Ratios are arbitrarily set low at 2.03.0 in our institution. Oral anticoagulation is also continued in parallel to long-term intravenous prostacyclin. In special circumstances, anticoagulation is performed with subcutaneous low molecular weight heparins, but data concerning the "safety" of such regimens are lacking. The question of whether long-term "platelet antagonists", such as the glycoprotein receptor IIb IIIa antagonists or ticlopidine, will be as effective as coumadine, needs to be answered as our knowledge concerning the role of platelets in PPH increases and randomized studies are performed. Future directions of therapy Therapy in the future will depend on new insights into the pathogenesis of pulmonary hypertension e.g. the understanding of molecular mechanisms underlying familial PPH ; and the delineation of pathologically distinct entities within the clinical appearance of severe pressure elevation in the pulmonary vascular bed. Among the currently available drugs, the vasodilator prostacyclin has been outstanding in its capacity: 1 ; to lower PVR acutely; and 2 ; to improve exercise capacity and outcome in chronically treated patients, despite the lack of an acute haemodynamic response to the drug. However.
