Oral fluid obtained by use of commercial collection devices offers several advantages compared to venepuncture less invasive, less painful, less expensive and safe ; . Moreover using oral fluid for HBV DNA detection could be useful for epidemiological purposes, especially in patient groups where the collection of serum is difficult intravenous drug users, children ; and for monitoring antiviral treatment. Methods: In collaboration with the university hospital of Antwerp, 146 paired samples serum and oral fluid ; are collected from patients attending the hospital for hepatitis B 73 HBV positive ; and for other pathologies 73 HBV negative ; . Oracol collection devices are used for the collection of the oral fluid samples and the quality of these samples is assessed by an IgG quantification test. The serum samples are tested by an in house HBV nested PCR. Primers towards the beta-actin gene are used to monitor inhibition during extraction and amplification steps. For the optimization of the HBV detection in oral fluid, different parameters were evaluated: three extraction kits, extraction and amplicon volumes, concentrations of beta-actin primers. Results: We obtained the best results with the use of the High Pure Template Preparation kit for extraction and the lowering of the concentration of beta-actin primers from 25 pmol to 5 pmol in a 50 mix. Five semi-logarithmic dilutions of HBV DNA-containing plasma 3000 geq ml ; in negative oral fluid were used to assess the sensitivity of the optimized assay. The 95% detection limit of our results was a concentration of 457 geq ml. Conclusion: We developed a HBV nested PCR for oral fluid with a sensitivity of 457 geq ml. For the validation of this newly developed nested PCR, 73 HBV positive and 73 HBV negative oral fluid samples will be tested in order to determine the clinical sensitivity and specificity. 84.
